Caring for dialysis patients places special demands on nursing and medical care. Our outpatient nursing service supports patients both during home dialysis (eg peritoneal dialysis) and in collaboration with outpatient or inpatient dialysis centers.
We offer:
- Home dialysis support:
Guidance, help and support during peritoneal dialysis or home hemodialysis. Monitoring procedures, hygiene and safety. - Medical assistance:
Monitoring vital signs (blood pressure, pulse, weight, fluid balance), taking medications as prescribed, wound care (eg, bypass surgery). - Organization and coordination:
Coordinate with physicians, dialysis centers and hospitals, scheduling and communication. - Training and consulting:
Training patients and relatives in safe home dialysis, hygiene rules, and emergency measures. - Psychosocial support:
Support in everyday life, counseling services, support and assistance to family members.

Who is suitable for dialysis/home care?
Our services are aimed at:
- Patients who require dialysis regularly (usually 3–4 times a week);
- People who have chosen home dialysis and need professional support;
- Family members who need help and professional support during home dialysis;

Financing and covering expenses:
- Long-term care insurance (SGB XI):
Care services such as basic care, support with activities of daily living or counseling may be covered through the long-term care insurance fund (depending on the level of care). - Health insurance (SGB V):
Medical services such as medication, bandage changes or bypass surgery are covered by the health insurance company if they are prescribed by a doctor (home care in accordance with § 37 SGB V). - Combination:
Dialysis patients often receive care through a combination of long-term care and health insurance. We provide personalized advice and handle claims processing and billing directly to insurance companies.

Our process
- Consultation and needs analysis:
Together we will determine what type of dialysis you need (home or in-center dialysis) and what support will be helpful. - Medical prescription/application:
We require a doctor’s prescription to receive medical and nursing services. We will help you organize everything and establish interaction with your insurance company. - Individual care plan:
We will work with you and, if necessary, your nephrologist, to develop a personalized plan of care and support. - Regular care:
Our specialists accompany home dialysis, monitor processes, document indicators and ensure safety and hygiene. - Individual setup and consultation:
- We regularly review the situation and adapt care to your medical and personal needs.
